Mental Health Targeted Case services are designed to support adults with gaining access to a wide range of behavioral health services.  Case managers assist with linkages to federal, state, and/or local resources and provide consumers with the individualized tools they need to reduce the need for a higher level of care.  These services aim to support an increase in community stability and tenure through referral to behavioral health treatment and support services. Case managers support the attainment of these goals, through the core components of Mental Health Targeted Case Management services which include but are not limited to the following:

  • Comprehensive Assessment and Periodic Reassessment
  • Development and Periodic Revision of a Specific Care Plan
  • Referral and Related Activities
  • Coordination of Services
  • Participant Advocacy
