Targeted Case Management is a primary, direct service provided to adults or children to support them in gaining access to needed medical, mental health, social, educational, and other services. TCM is designed to increase participants with moving toward their individualized goals while living in the community.  Services are offered through a variety of levels of care (LOC) to support  navigation through complex behavioral health challenges. Our team of qualified case managers facilitate the community-based assessment and planning process by working with the individuals, families, service providers, and other agencies to create a care plan based on individualized needs.  In addition, the case management team assists individuals and families with navigating the service system and reducing gaps in care, which increases feelings of recovery, independence, and the ability to manage the symptoms of their illness.
